Splinter hemorrhages are a sign of infective endocarditis when they occur more than 2mm from the end of the nail, usually in the middle of the nail bed or proximally.Distil splinter hemorrhages are thought to be due to trauma and of little clinical significance.However recent interest reveals that splinter hemorrhages within 2mm from the end of the nail bed are actually located in the capillary nail band at the periphery of the nail and are associated with periodontal disease, diabetes and obesity, which raises the question, are splinter hemorrhages markers not only of cardiovascular pathology but of micro-vascular and inflammatory disease as well?
