目的评价CT/MR液体衰减反转恢复(fluid attenuated inversion recovery,FLAIR)序列对急性蛛网膜下腔出血(subarachnoid hemorrhage,SAH)的临床应用价值。方法 40例无颅脑疾病者和82例急性SAH患者在发病3天内行头MRI检查,观察急性SAH在FLAIR序列上的信号表现及CT表现,将两种检查结果进行比较。对颅脑正常者和SAH的FLAIR图像进行双盲分析,以评价FLAIR对SAH的诊断价值。结果 MR FLAIR序列对急性SAH的显示率为97.5%(80/82),明显高于CT显示率(79.3%,65/82)。结论 MR FLAIR序列对急性SAH的诊断优于CT,应作为常规检查方法。
Objective To evaluate the clinical value of fluid attenuated inversion recovery (FLAIR) sequence in the diagnosis of subarachnoid hemorrhage (SAH). Methods 40 patients with craniocerebral disease and 82 patients with acute SAH underwent head MRI examination within 3 days after onset. The signal and CT findings of acute SAH in FLAIR sequence were observed. The results of the two tests were compared. A double-blind analysis of FLAIR images of normal brain and SAH was performed to evaluate the diagnostic value of FLAIR for SAH. Results The MR FLAIR sequence showed 97.5% (80/82) of acute SAH, which was significantly higher than that of CT (79.3%, 65/82). Conclusion MR FLAIR sequence is superior to CT in the diagnosis of acute SAH and should be used as routine examination.
