Primary malignant melanoma of the esophagus (PMME) is an extremely rare, but highly aggressive tumor.Up to 2011, only more than 300 cases had been reported worldwide in the English literature. Most patients were at an advanced stage when diagnosed. How to increase the diagnostic accuracy of PMME patients in early stage is even a challenge. The accuracy of diagnosis from endoscopy and biopsy is approximately 80%, the accurate diagnosis could not be made because of the absence of melanin granules in poorly differentiated carcinoma or some other reasons. The endoscopic ultrasound was performed to examine primary malignant of the esophagus in 1996.The tumor was depicted as a hypoechoic or mixed echogenicity welldemarcated mass existed in the second to third mural layer at endoscopic ultrasound. We present two cases of PMME.Both of them were not diagnosed as PMME by endoscopy and biopsy, but combined with endoscopic ultrasound, the clinical diagnosis were made.We also found that the internal architecture was no any bloodstream.Endoscopic ultrasound can confirm PMME further, especially in the case of accurate diagnosis could not be made by endoscopy and biopsy.
