主诉:面部红斑乏力劳累7个月,伴关节疼痛一周.病史,查体及实验室检查:患者女性32岁,患者7个月前面部,尤其是鼻梁及两侧面颊部起对称性红斑,无痛痒不适,未做处理,2月前口腔出现溃疡,自行愈合,后间断发作,1月前头顶部脱发,逐渐加重,一周前四肢关节出现疼痛不适,呈阵发性发作.病程中,一般情况可.入院后,身上开始起水疱.之前无明显诱因感双手背、指关节、膝关节疼痛,无明显规律.体格检查:双侧面部对称分布不规则核桃大小暗褐色斑片,右侧耳廓糜烂渗出倾向,头顶部斑片状脱发,躯干弥漫分布米粒至黄豆大小丘疹、丘疱疹、水疱,部分结痂坏死.双手关节处、甲周见红斑,口腔散在溃疡.四肢关节有压痛.
