Self-management is vital to maintaining heart failure (HF) patients health.Globally, health systems face significant barriers that limit their ability to provide ongoing health monitoring and behavior change services.In this ongoing type 2 translational research project, we are developing and evaluating telecommunication and informatics strategies for effectively involving informal caregivers, or "CarePartners", in patients HF self-care in an effort to fill gaps in traditional care management.The CarePartner program includes weekly telephonic HF assessment and behavior change calls to patients, with immediate tailored feedback to the patient and follow-up reports to their CarePartners via email and a website.A primary goal is to identify self-care problems and symptoms early to prevent acute crises.
