目的:探讨超促排卵周期胚胎移植日血清雌二醇(E2)浓度的变化对体外受精-胚胎移植(IVF-ET)治疗结局的影响,为制定个体化黄体支持方案提供参考。方法:回顾性分析125个IVF-ET周期,按ET日血清E2浓度变化分为4组:A组:24个治疗周期,ET日血清E2浓度/HCG注射日E2浓度<0.4;B组:54个周期,0.4≤ET日E2/HCG日E2<0.7;C组:32个周期,0.7≤ET日E2/HCG日E2<1;D组:15个周期,ET日E2/HCG日E2≥1。结果:C组胚胎种植率和妊娠率最高,A组种植率和妊娠率均显著降低(P<0.05)。结论:IVF-ET采用长方案控制性超排卵(COH),移植日E2水平过高或过低均影响治疗结局,监测E浓度对制定个体化黄体支持方案具有重要的临床价值。
OBJECTIVE: To investigate the effect of serum estradiol (E2) concentration on the outcome of IVF-ET in ovariectomized embryo transfer (IVF-ET) and to provide a reference for the development of a personalized luteal support protocol. Methods: A total of 125 cycles of IVF-ET were retrospectively analyzed. The serum concentrations of E2 in ET were divided into 4 groups: group A: 24 cycles, E2 level on ET day / E2 level on day 0 of HCG injection; E2≤0.1 day E2 / HCG day E2 <0.7; C group: 32 cycles, 0.7≤ET day E2 / HCG day E2 <1; D group: 15 cycles, ET day E2 / HCG day E2≥1 . Results: The implantation rate and pregnancy rate of group C were the highest, and the implantation rate and pregnancy rate of group A were significantly decreased (P <0.05). CONCLUSIONS: Long-term regimen controlled ovarian hyperstimulation (COH) is an effective treatment for IVF-ET. The high or low E2 level on the day of transplantation affects the outcome of treatment. Monitoring E concentration has important clinical value in the development of personalized luteal support protocol.
