同型半胱氨酸(homocysteine,Hcy)为一种含巯基的氨基酸,是甲硫氨酸的代谢中间产物.大量研究证实,Hcy与心脑血管疾病、肝脏疾病、肾脏疾病、老年痴呆症等多种疾病有关,是导致心脑血管疾病的一个独立危险致病因子.同型半胱氨酸浓度监测主要应用于临床冠状动脉粥样硬化和心肌梗塞患者,它的浓度升高程度与疾病的危险性成正比.半胱氨酸(cysteine,Cys)、半胱氨酰甘氨酸(cysteinylglycine,CysGly)、谷胱甘肽(glutathione,GSH)与Hcy代谢密切相关,为一类氨基硫醇物,是体内重要的胞外氧化还原系统,其中任一物质有质和量的改变均会使其他物质发生代谢紊乱.Cys是Hcy的转硫化产物,其结构和生化特性与Hcy相似.Cys升高与心、脑血管疾病、血管闭塞性疾病密切相关.另外根据同型半胱氨酸的代谢特点,维生素B12、叶酸缺乏症以及遗传性甲基四氢叶酸转甲基酶、胱硫醚-合成酶基因缺陷也与同型半胱氨酸浓度有密切关系,建立准确快速的同型半胱氨酸及其硫醇物检测方法是心脑血管及相关疾病预防和治疗的基础.
