目的:评价临床体检(GBE)、超声检查(US)与乳腺钼靶X线检查(MAM)在乳腺癌筛查中的应用价值.
方法:选择2011-2012年参加CBE、US与MAM筛查乳腺癌的35-59岁妇女作为研究对象,收集筛查个案记录,以病理组织学检查作为"金标准",计算三种筛查方法单独应用与联合应用筛查诊断乳腺癌的精确性.
结果:共27991人纳入研究,检出乳腺癌161例,患病率为575.2/10万;检出乳腺良性疾病11335例,患病率为40.5%.161例乳腺癌中,CBE检出68例;US检出93例;MAM检出116例.单独应用CBE的敏感度为42.2%;US为57.8%;MAM为72.0%;CBE与US联合为68.9%;CBE与MAM联合为80.7%:US与MAM联合为85.1%;CBE、US和MAM三种方法联合的敏感度最高(90.1%),但特异度最低(89.9%).
结论:在卫生资源有限情况下,CBE与US结合进行乳腺癌初筛,必要时进行MAM检查,可以进一步提高筛查乳腺癌的敏感性.
