Background Survival with spontaneous circulation after out-of-hospital cardiac arrest(OHCA) is often associated with neurological and other sequelae. Methods Case report. Results We describe a patient who survived after prolonged cardiac arrest(total duration of cardiac arrest was 78 min) due to Brugada sydrome and got a full recovery without any neurologic impairment. Conclusions Prolonged resuscitation is necessary in OHCA due to Brugada sydrome.Therapeutic hypothermia may be helpful for converting cardiac arrest with Brugada sydrome to restoration of spontaneous circulation.Prolonged CPR, therapeutic hypothermia and hyperbaric oxygen may be effective for neuroprotection in cardiac arrest with asystole due to Brugada sydrome.
