Drug discovery and drug development are facing a sustainability crises. The conventional basic biomedical research at molecular and cellular level is for knowledge accumulation that can be rarely translated into practical applications. The perceived importance of basic research has tilted toward lopsided biomedical situation that blur the focus of logic and trivialize the purpose with gross neglection of fundamentals, such as how to draw a dose-effect curve, what is the additive effect and what is synergy? Without clear definition and technical know-how on the fundamentals, it compromises the credibility and accountability of biomedical sciences.A typical drug development from bench work to a new drug takes over 6 years with over >$500M cost but with 90% attrition rate.In the field of cancer, we are still mainly using the drugs that were developed over 30 or 50 years ago, and the age-specific cancer death rates are still what it was in 1970s or even 1950s.
