Bimodal relapse patterns have now been identified in 13 independent breast cancer databases from the US,Europe and Asia.As my colleagues and I have reported,interpretation of this pattern strongly suggests that metastatic breast cancer growth includes periods of temporary dormancy at the single cell level and prior to angiogenesis.Furthermore,surgery to remove the primary tumor induces cell division and (especially for premenopausal node positive patients) stimulates angiogenesis of dormant distant micrometastases in over half of all relapses.This theory seems to explain a wide variety of clinical features in breast cancer.These include 1) paradoxical findings from early detection trials for women age 40-49,2) why adjuvant chemotherapy works best by far for premenopausal node-positive patients,3) the excess mortality of African-Americans over European-Americans,4) very poor outcome of breast cancer in Nigeria,5) heterogeneity of breast cancer,6) "aggressiveness" of breast cancer in young women,and 7) similar reports from pre-modern physicians starting 2000 years ago and continuing until Halsted.
