目的观察不同定位方式下靶肌肉注射A型肉毒素治疗儿童痉挛型脑瘫上肢功能的疗效。方法选择痉挛性脑瘫患儿50例,分为2组,治疗组为肌电图引导下定位,对照组为反向徒手牵拉定位,剂量均3U/kg。注射后配合康复训练,采用改良Ashworth痉挛量表评定肌痉挛程度,随访1、3个月的疗效。结果各组治疗前后Ashwort分级减轻(P<0.05),但不同定位方式治疗后Ashwort分级差别无统计学意义(P>0.05)。结论 BTX-A可改善痉挛型脑瘫患儿上肢功能,但不同定位方式下治疗后疗效指标差别无统计学意义。
Objective To observe the efficacy of targeted intramuscular injection of botulinum toxin (A) in children with spastic cerebral palsy on the function of the upper extremities under different targeting methods. Methods Fifty children with spastic cerebral palsy were selected and divided into two groups. The treatment group was located under the guidance of EMG, and the control group was reverse hand pulling and positioning, the dosage was 3U / kg. Combined with rehabilitation training after injection, using modified Ashworth spasm scale to assess the degree of muscle spasm, followed up 1,3 months of efficacy. Results Ashwort grade was reduced before and after treatment in each group (P <0.05), but there was no significant difference in Ashwort grade after treatment (P> 0.05). Conclusion BTX-A can improve the function of upper extremity in children with spastic cerebral palsy, but there is no significant difference in curative effect between different positioning methods after treatment.
