目的探讨应用三维CT血管造影(CTA)在神经外科疾患术前诊断及评估中的作用。方法对40例患者(23例脑血管病、4例颈部血管性病变和13例颅底或高血供脑瘤)进行了43次CTA和26次数字减影血管造影(DSA)检查。结果CTA的诊断与DSA结果相同并被手术证实。CTA不仅发现8例动脉瘤和8例动静脉畸形,并且显示了这些病变的三维构造和毗邻的骨结构。CTA准确地显示了颈动脉、椎动脉的狭窄并可显示颅内肿瘤与毗邻血管及颅骨的关系。结论CTA具有简单、快速、无创和可靠等优点,适合于脑血管病、血供丰富的颅内肿瘤及颅底肿瘤的术前诊断和评估,对颅内动脉瘤的诊断具有极高的准确性
Objective To explore the application of three-dimensional CT angiography (CTA) in preoperative diagnosis and evaluation of neurosurgical diseases. Methods Forty-three CTAs and 26 digital subtraction angiography (DSA) examinations were performed in 40 patients (23 with cerebrovascular disease, 4 with cervical vessel disease, and 13 with skull base or hypervascularity). Results The diagnosis of CTA was the same as that of DSA and was surgically confirmed. CTA not only found 8 aneurysms and 8 arteriovenous malformations, but also showed the three-dimensional structure of these lesions and adjacent bone structures. CTA accurately shows the stenosis of carotid and vertebral arteries and can show the relationship between intracranial tumors and adjacent blood vessels and skull. Conclusion CTA has the advantages of simple, rapid, noninvasive and reliable. It is suitable for preoperative diagnosis and evaluation of cerebrovascular disease, blood-rich intracranial tumors and skull base tumors, and has very high accuracy for the diagnosis of intracranial aneurysms
