安氏Ⅱ~1与安氏Ⅱ~2错牙合下颌骨生长旋转的研究

目的:探讨安氏Ⅱ类1分类及2分类不同CVM分期下颌骨旋转的差异。方法:安氏Ⅱ类1分类422例(男197例,女225例),2分类241例(男91例,女150例),拍摄初诊头颅侧位片,按颈椎CVM分期及性别进行分组,测量下颌真性旋转角(SN-Xi Pm)、下颌平面角(SN-Go Gn)、牙合平面角(SN-OP),后前面高比(S-Go/N-Me(%)),通过SPSS16.0软件分析比较二者的生长发育差异。结果:安氏Ⅱ类1分类与2分类男性下颌真性旋转的趋势相同;女性在各CVM分期呈相反的变化,男性安氏Ⅱ类1分类下颌平面角较2分类者大1.58°~3.51°,在CS4期(P=0.021),CS5期(P=0.04)差异均有统计学意义;女性在CS1-CS2期,安氏Ⅱ类1分类下牙合平面角平均减小3.09°,之后有所增加,但变化较小;2分类下牙合平面角CS2-CS3期平均减小2.51°,在CS3-CS4期增加约2.68°,其余各期变化不明显。男性安氏Ⅱ类1分类牙合平面角较2分类者小,在CS5期差异有统计学意义;女性安氏Ⅱ类1分类的牙合平面角在CS1-CS6期逐渐减小,2分类牙合平面角在CS1-CS2期大幅增加,在CS2-CS3期又逐渐减小,之后变化幅度较小,CS5,CS6期安氏Ⅱ类1分类与2分类的差异有统计学意义。结论:安氏Ⅱ类1分类与2分类男性下颌真性旋转方向相同,女性则相反.与安氏Ⅱ类2分类相比,安氏Ⅱ类1分类OP-SN角较小,Go Gn-SN角较大,性别间无差异。 Objective: To investigate the difference of mandibular rotation between Class Ⅱ division 1 and type 2 CVM staging. Methods: A total of 422 cases of Class Ⅱ division 1 were enrolled in this study, including 197 males and 225 females. Two groups of 241 males (91 males and 150 females) were enrolled in this study. SN-Xi Pm, SN-Go Gn, SN-OP and S-Go / N-Me (% SPSS16.0 software analysis and comparison of the growth and development differences between the two. Results: The trend of true rotation of Class Ⅱ division 1 was the same as that of the classification of 2 males. The female showed the opposite change in each stage of CVM. The level of mandibular plane angle of Class Ⅱ division 1 was 1.58 ° ~ 3.51 ° higher than that of category 2, There were significant differences in CS4 (P = 0.021) and CS5 (P = 0.04) between the two groups. The mean reduction of the occlusal plane angle in women with CS1-CS2 and Class Ⅱ division 1 was 3.09 ° But the change was small. The average CS2-CS3 phase of occlusal plane angle in 2 categories decreased by 2.51 ° on average, and increased by 2.68 ° in CS3-CS4 stage, while the other stages had no obvious changes. The prevalence of Class Ⅱ division 1 malocclusion plane was smaller than that of the 2 classification group, and the difference was statistically significant at CS5 stage. The level of occlusion plane angle of Class Ⅱ division 1 lesion decreased gradually in CS1-CS6 stage, The co-plane angle increased significantly in CS1-CS2 phase and gradually decreased in CS2-CS3 phase, but the change range was smaller after CS2-CS3. There was significant difference between CS2 and CS6 CS2 class 1 classification and 2 classification. CONCLUSIONS: Class Ⅱ division 1 and class 2 males have the same direction of true males’ true rotation, while female females have the opposite trend.Compared with the class Ⅱ division 2 class, Class Ⅱ division 1 has a smaller OP-SN angle and a smaller Go-Gn-SN angle Larger, no difference between the sexes.
