目的掌握甘肃省陇西县人群γ-干扰素释放试验检测结核分枝杆菌潜伏感染流行病学特征及影响因素。方法通过问卷调查收集研究对象基本信息。采集血液样本,采用γ-干扰素释放试验(IGRA)判定是否结核分枝杆菌潜伏感染。采用SPSS 15.0软件进行数据分析。感染阳性率的比较用χ2检验。对χ2检验中有统计学意义的因素进行二元多因素logistic回归分析。结果本研究5 527例,IGRA阳性957例,结核分枝杆菌潜伏感染率为17.3%。男性结核分枝杆菌潜伏感染率为21.2%,女性结核分枝杆菌潜伏感染率为14.4%,差别有统计学意义(χ2=43.585,P<0.05)。结核分枝杆菌潜伏感染率随着年龄的增大呈线性上升趋势,差异有统计学意义(χ2=545.610,P<0.05)。不同职业间结核分枝杆菌潜伏感染率差异有统计学意义(χ2=395.949,P<0.05)。性别、年龄、职业、吸烟与否、饮酒与否和食用豆制品频率不同情况下,结核分枝杆菌潜伏感染率差异有统计学意义(P<0.05)。logistic回归分析结果显示男性、年龄增大、吸烟为结核分枝杆菌潜伏感染的危险因素。结论通过γ-干扰素释放试验检测5 527例甘肃省陇西县人群,掌握了结核分枝杆菌潜伏感染流行病学特征及影响因素。
Objective To investigate the epidemiological characteristics and influencing factors of latent infection of Mycobacterium tuberculosis detected by γ-interferon release test in Longxi County, Gansu Province. Methods The questionnaires were used to collect the basic information of the subjects. Blood samples were taken and IFN-γ release assay (IGRA) was used to determine if there was latent infection with M. tuberculosis. SPSS 15.0 software was used for data analysis. The positive rate of infection was compared with χ2 test. Two-factor logistic regression analysis of statistically significant factors in the χ2 test. Results A total of 5 527 cases of IGRA were enrolled in this study, 957 cases of IGRA were positive, and the latent infection rate of Mycobacterium tuberculosis was 17.3%. The latent infection rate of M. tuberculosis was 21.2% in men and 14.4% in women, the difference was statistically significant (χ2 = 43.585, P <0.05). The latent infection rate of Mycobacterium tuberculosis increased linearly with age, the difference was statistically significant (χ2 = 545.610, P <0.05). There were significant differences in the latent infection rates of Mycobacterium tuberculosis among different occupations (χ2 = 395.949, P <0.05). There were significant differences in the latent infection rates of Mycobacterium tuberculosis between sex, age, occupation, smoking or not, drinking or not and the frequency of eating soy products (P <0.05). Logistic regression analysis showed that men, age, smoking as a risk factor for latent infection of M. tuberculosis. Conclusion The epidemiological characteristics and influential factors of latent infection of Mycobacterium tuberculosis were mastered by γ-interferon release test in 5 527 cases of Longxi County, Gansu Province.
