目的探讨血清可溶性血管细胞粘附分子-1(SVCAM-1)水平和对氧磷酯酶1(PON1)与老年2型糖尿病心脑血管并发症的关系。方法将62例老年2型糖尿病患者分为心脑血管并发症组(32例),无心脑血管并发症组(30例),并选取36例非糖尿病的健康者为对照组。测定老年2型糖尿病各组患者及健康老年人血清对氧磷酯酶1(PON1)、可溶性血管细胞粘附分子-1(SVCAM-1)。结果 2型糖尿病心脑血管并发症组SVCAM-1高于无心脑血管并发症组和对照组,无心脑血管并发症组高于对照组;2型糖尿病心脑血管并发症组PON1低于无心脑血管并发症组和对照组,无心脑血管并发症组低于对照组。血清SVCAM-1与收缩压(SBP)、舒张压(DBP)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白(LDL-C)显著正相关,与高密度脂蛋白(HDL-C)、PON1显著负相关。结论 SVCAM-1和PON1在老年2型糖尿病心脑血管并发症的发生中起重要作用,血管内皮功能障碍可能参与其中。
Objective To investigate the relationship between serum soluble vascular cell adhesion molecule-1 (SVCAM-1) and paraoxonase 1 (PON1) and cardiovascular and cerebrovascular complications in type 2 diabetic elderly patients. Methods Sixty-two elderly patients with type 2 diabetes mellitus were divided into cardiovascular and cerebrovascular complications group (n = 32) and no cardiovascular and cerebrovascular complications group (n = 30). 36 non-diabetic healthy subjects were selected as control group. Serum paraoxonase 1 (PON1) and soluble vascular cell adhesion molecule-1 (SVCAM-1) levels were measured in elderly patients with type 2 diabetes mellitus and healthy elderly. Results SVCAM-1 in type 2 diabetes mellitus patients with cardiovascular and cerebrovascular complications was higher than those without cardiovascular complication and control group, and no cardiovascular complication group was higher than those in control group. PON1 was lower in type 2 diabetes mellitus patients with cardiovascular and cerebrovascular complications No cardiovascular and cerebrovascular complications and control group, no cardiovascular and cerebrovascular complications group than in the control group. Serum SVCAM-1 was significantly and positively correlated with SBP, DBP, TG, TC and LDL-C, and positively correlated with high density lipoprotein (HDL) -C), PON1 was significantly negatively correlated. Conclusion SVCAM-1 and PON1 play an important role in the pathogenesis of cardiovascular and cerebrovascular complications in type 2 diabetes mellitus in elderly patients. Vascular endothelial dysfunction may be involved.
