目的探讨几丁质酶-3样蛋白-1(chitinase-3-like-1 protein,YKL-40)在心房颤动(房颤)发病机制中的作用。方法选择在广州市番禺区何贤纪念医院心内科住院的房颤患者71例(其中阵发性房颤组22例,持续性房颤组30例,永久性房颤组19例)为研究对象。另外,选择同期年龄匹配的健康体检者20名为对照组。采用酶联免疫吸附试验(ELISA)方法检测各组血浆YKL-40浓度并进行比较。结果各组间基线资料比较,差异无统计学意义(P>0.05)。永久性房颤组、持续性房颤组及阵发性房颤组血浆YKL-40浓度均显著高于对照组,差异有统计学意义[(4.21±0.62)μg/L、(3.72±0.63)μg/L、(3.29±0.75)μg/L vs.(2.79±0.56)μg/L,P<0.05];持续性及永久性房颤组血浆YKL-40浓度高于阵发性房颤组,差异有统计学意义(P<0.05);永久性房颤组YKL-40浓度高于持续性房颤组,差异有统计学意义(P<0.05)。结论YKL-40可能参与房颤的发生、发展。
Objective To investigate the role of chitinase-3-like-1 protein (YKL-40) in the pathogenesis of atrial fibrillation (AF). Methods A total of 71 patients (including 22 patients with paroxysmal atrial fibrillation, 30 patients with persistent atrial fibrillation and 19 patients with permanent atrial fibrillation) who were hospitalized in Department of Cardiology, He Xian Memorial Hospital, Panyu District, Guangzhou City were selected as research objects . In addition, 20 healthy subjects of the same age were selected as the control group. The concentration of plasma YKL-40 in each group was detected by enzyme linked immunosorbent assay (ELISA) and compared. Results There was no significant difference in baseline data between groups (P> 0.05). The concentrations of plasma YKL-40 in permanent atrial fibrillation group, persistent atrial fibrillation group and paroxysmal atrial fibrillation group were significantly higher than those in control group [(4.21 ± 0.62) μg / L, (3.72 ± 0.63) (3.29 ± 0.75) μg / L vs. (2.79 ± 0.56) μg / L, P <0.05]. The concentrations of plasma YKL-40 in patients with persistent and permanent atrial fibrillation were higher than those in patients with atrial fibrillation, The difference was statistically significant (P <0.05). The concentration of YKL-40 in permanent atrial fibrillation group was higher than that in persistent atrial fibrillation group, the difference was statistically significant (P <0.05). Conclusion YKL-40 may be involved in the occurrence and development of atrial fibrillation.
