目的探讨CD64指数在鉴别系统性红斑狼疮(SLE)继发感染和病情活动中的意义。方法 87例SLE患者分为细菌感染组(A组,32例)和非细菌感染组(B组,55例);后者依SLE疾病活动性指数(SLE DAI)分为SLE活动组(B1组,SLE DAI≥8分,25例)与SLE稳定组(B2组,SLE DAI<8分,30例)。另设健康对照组(C组,30例)。采用流式细胞术检测外周血中性粒细胞、单核细胞及淋巴细胞表面CD64的平均荧光强度,计算出CD64指数,与感染指标和病情活动指标进行相关性分析。结果 A组CD64指数显著高于B1组、B2组和C组(7.90±3.43vs.2.44±0.98、1.49±0.64和1.45±0.61)(P<0.01)。A组CD64指数与WBC数、中性粒细胞数、血沉、C反应蛋白、降钙素原水平呈显著正相关(r=0.483,r=0.535,r=0.601,r=0.676,r=0.724,P<0.01),而与SLE DAI、抗核抗体、抗ds-DNA抗体、补体3、补体4水平之间均无显著相关性(P>0.05);B1组CD64指数与上述所有指标间皆无显著相关性(P>0.05)。结论 CD64指数可作为SLE继发细菌感染和病情活动的鉴别指标。
Objective To investigate the significance of CD64 index in the identification of secondary infection and disease activity in systemic lupus erythematosus (SLE). Methods 87 patients with SLE were divided into bacterial infection group (group A, 32 cases) and non-bacterial infection group (group B, 55 cases). The latter group was divided into SLE group (group B1) according to SLE DAI , SLE DAI≥8 points, 25 cases) and SLE stable group (B2 group, SLE DAI <8 points, 30 cases). Another healthy control group (C group, 30 cases). The average fluorescence intensity of CD64 on peripheral blood neutrophils, monocytes and lymphocytes was detected by flow cytometry, and the CD64 index was calculated, and the correlation between infection index and disease activity index was analyzed. Results The CD64 index of group A was significantly higher than that of group B1, group B2 and group C (7.90 ± 3.43 vs.2.44 ± 0.98, 1.49 ± 0.64 and 1.45 ± 0.61 respectively) (P <0.01). The CD64 index of group A was positively correlated with the number of WBC, neutrophil, ESR, C-reactive protein and procalcitonin (r = 0.483, r = 0.535, r = 0.601, r = 0.676, r = 0.724, P <0.01), but no significant correlation with SLE DAI, antinuclear antibody, anti-dsDNA antibody, complement 3 and complement 4 (P> 0.05). There was no difference between CD64 index of B1 group and all above indexes Significant correlation (P> 0.05). Conclusion CD64 index can be used as a secondary indicator of SLE bacterial infection and disease activity.
