选取2013年8月~2015年10月在我院急诊内科住院的糖尿病伴急性脑梗死患者120例,随机分为3组:对照组单纯应用降糖、调脂、降压、抗凝、护脑及对症等常规治疗;治疗组在常规治疗基础给予脑心通胶囊治疗;联合用药组在常规治疗基础上给予川青~联合脑心通胶囊治疗。观察三组患者治疗前后临床疗效、血液流变学、血脂代谢指标、炎症因子的变化。结果治疗组治疗前后神经功能缺损评分明显改善,且优于对照组(均P<0.05),联合用药组改善更明显(均P<0.05),治疗组和联合用药组总有效率均高于对照组(均P<0.05),联合用药组与治疗组总有效率差异无统计学意义(P>0.05),治疗组对降低患者纤维蛋白原、血浆粘度、TNF-α作用明显强于对照组(均P<0.05);联合用药组降低患者纤维蛋白原、血浆粘度、TNF-α作用较治疗组更强(均P<0.05)。血脂代谢方面,联合用药组与治疗组降低胆固醇及三酯酰甘油作用相当,两者均高于对照组(均P<0.05)。结论脑心通胶囊对糖尿病伴急性脑梗死患者有较好的疗效,联用川芎嗪注射液的疗效更佳,其可能机制为川芎嗪注射液联合脑心通胶囊能降低患者血液纤维蛋白原、血浆粘度,降低TNF-α炎症因子水平。
120 patients with diabetes mellitus and acute cerebral infarction hospitalized in emergency department of our hospital from August 2013 to October 2015 were randomly divided into 3 groups: control group, which was used to decrease blood sugar, lipid-lowering, blood-pressure-lowering, anticoagulation, And symptomatic treatment. The treatment group was given Naoxintong capsule on the basis of conventional treatment. The combination treatment group was given Chuanqing ~ combined Naoxintong capsule on the basis of routine treatment. The clinical curative effect, hemorheology, blood lipid metabolism index and inflammatory factors in three groups were observed before and after treatment. Results The score of neurological deficits in treatment group before and after treatment was significantly improved, which was significantly better than that in control group (all P <0.05), and that in combination group was significantly improved (all P <0.05). The total effective rate in treatment group and combination group was higher than that in control group (P 0. 05). There was no significant difference in the total effective rate between the combination therapy group and the treatment group (P 0. 05). The treatment group was significantly more effective in reducing fibrinogen, plasma viscosity and TNF-α in the patients than in the control group (All P <0.05). The combined treatment group decreased fibrinogen and plasma viscosity and the effect of TNF-α in the patients was better than that in the treatment group (all P <0.05). Blood lipid metabolism, the combination group and the treatment group to reduce cholesterol and triglyceride quite the same, both were higher than the control group (P <0.05). Conclusion Naoxintong capsule has a good curative effect on patients with diabetes mellitus and acute cerebral infarction. The combined effect of ligustrazine injection is better. The possible mechanism is that ligustrazine injection combined with Naoxintong capsule can reduce blood fibrinogen, Plasma viscosity reduces the level of TNF-α inflammatory cytokines.
