目的:分析医改后,我国基层医疗机构卫生技术人员配置的变化趋势及存在的问题,为政府优化卫生资源配置政策提供理论基础。方法:采用非抽样调查的全面定期统计报表调查方式,对基层医疗机构卫生技术人员按地理面积分布的变化趋势进行统计描述分析。结果:2009—2012年,基层医疗机构卫生技术人员按地理面积分布整体涨幅2.22%,东、西部地区涨幅分别为7.87%和9.09%,西部地区绝对值显著低于其他地区。社区卫生服务机构涨幅44.38%,乡镇卫生院降幅25.81%,执业医师涨幅12.90%,注册护士涨幅28.30%。结论:基层医疗机构卫生技术人员整体配置呈上升趋势,但城乡差距拉大,各地区配置水平差异显著,政府应加强农村及西部地区基层医疗机构卫生技术人员的配置,加强护理人员队伍建设。
OBJECTIVE: To analyze the changing trends and existing problems of the distribution of health technicians in primary medical institutions in China after medical reform, and provide the theoretical basis for the government to optimize the allocation policies of health resources. Methods: Using non-sampling survey of comprehensive regular statistical reports survey, the primary health care institutions in the medical and health personnel according to the distribution of geographic distribution trends for statistical analysis. Results: From 2009 to 2012, the total number of health technicians in primary medical institutions rose 2.22% by geographical area. The gains in eastern and western regions were 7.87% and 9.09% respectively, while the absolute values in western regions were significantly lower than those in other regions. Community health service agencies rose 44.38%, township hospitals fell 25.81%, practicing physicians rose 12.90%, registered nurses rose 28.30%. Conclusion: The overall configuration of health technicians in grassroots medical institutions is on an upward trend. However, the gap between urban and rural areas is widening. There are notable differences in the level of configuration between regions. The government should strengthen the allocation of health technicians in primary and secondary medical institutions in rural and western regions and strengthen the contingent of nurses.
