目的采用锥形束CT(CBCT)测量技术定量评价健康成年人的髁突位置,并对髁突位置的三维测量方法进行探讨。方法筛选50例颞下颌关节健康的成年志愿者进行头颅CBCT扫描,使用Simplant 13.0软件测量髁突在矢状位、轴位和冠状位的相对位置,并进行双侧髁突位置对称性的比较。结果关节上间隙(冠状位)的均值为(4.04±0.93)mm;髁突半径值(轴位)均值为(54.14±2.77)mm;髁突在关节窝内的相对位置(矢状位)(AE’/AC)为(49.27±4.04)%;双侧髁突位置(关节上间隙、髁突半径值及髁突在关节窝内的相对位置)测量结果 ,差异均无统计学意义(t分别=-0.69、-1.93、0.74,P均>0.05)。结论健康成年人髁突位于关节窝中位,双侧髁突位置基本对称;CBCT影像测量是评价髁突位置的有效方法之一。
Objective To quantitatively evaluate the position of condyles in healthy adults by cone beam computed tomography (CBCT) technique and to investigate the three-dimensional measurement of condylar position. Methods Fifty adult volunteers with temporomandibular joint health were screened for cranial CBCT. The relative positions of the condyle in the sagittal, axial and coronal planes were measured by Simplant 13.0 software and the symmetry of bilateral condyles was compared. Results The mean value of the superior joint space (coronal) was (4.04 ± 0.93) mm. The average condylar radius value (axial) was (54.14 ± 2.77) mm. The relative position of the condyles in the joint socket (sagittal) (49.27 ± 4.04)% respectively. There were no significant differences in the measurement results of bilateral condyle position (joint gap, condyle radius and the relative position of condyle in the joint socket) = -0.69, -1.93, 0.74, P> 0.05). Conclusion The condyles of healthy adults are located in the middle of the joint fossa, and the position of the bilateral condyles is basically symmetrical. CBCT imaging is one of the effective methods to evaluate the condylar position.
