目的:探讨Caspase-9蛋白在甲状腺癌与腺瘤组织中的表达及其临床意义。方法:收集65例甲状腺癌、40例甲状腺腺瘤及40例正常甲状腺组织石蜡标本,应用免疫组织化学Envision法检测Caspase-9蛋白的表达。结果:甲状腺癌、甲状腺腺瘤及正常甲状腺组织中Caspase-9的阳性表达率分别为21.5%(14/65)、50.0%(20/40)和85.0%(34/40),呈依次升高趋势,三者阳性表达率比较差异有统计学意义,χ2=7.65,P<0.01。在甲状腺癌组织中,Caspase-9的阳性表达与AJCC分期(χ2=6.39)、组织分化(χ2=7.08)、颈部淋巴结转移(χ2=8.17)以及包膜浸润(χ2=9.25)密切相关,P<0.01;而与年龄、性别、肿瘤大小及病理分型无关。结论:Caspase-9蛋白在甲状腺癌和甲状腺腺瘤组织中均呈表达下调,在甲状腺癌中下调程度更严重,Caspase-9的表达下调不仅参与了甲状腺癌的形成过程,更在甲状腺癌浸润、转移等演进过程中发挥着重要的作用。
Objective: To investigate the expression of Caspase-9 protein in thyroid carcinoma and adenoma and its clinical significance. Methods: Paraffin-embedded specimens of 65 thyroid carcinomas, 40 thyroid adenomas and 40 normal thyroid tissues were collected. The expression of Caspase-9 protein was detected by immunohistochemistry Envision method. Results: The positive rates of Caspase-9 in thyroid cancer, thyroid adenoma and normal thyroid tissue were 21.5% (14/65), 50.0% (20/40) and 85.0% (34/40) respectively, Trend, the positive rate of the three was significantly different, χ2 = 7.65, P <0.01. In thyroid carcinoma, the positive expression of Caspase-9 was closely related to AJCC staging (χ2 = 6.39), tissue differentiation (χ2 = 7.08), cervical lymph node metastasis (χ2 = 8.17) and enveloping invasion (χ2 = 9.25) P <0.01; but not with age, gender, tumor size and pathological type. CONCLUSIONS: Caspase-9 protein is down-regulated in thyroid cancer and thyroid adenoma tissues, and is more down-regulated in thyroid cancer. The down-regulation of Caspase-9 is not only involved in the formation of thyroid cancer, but also in the invasion, Transfer and other evolutionary processes play an important role.
