目的:本研究旨在应用心肌组织超声背向散射定量新技术评价糖尿病患者心肌病变的特点和价值。方法:研究对象为经临床确诊为20例2型糖尿病患者。另设20例为正常对照组。每个切面分成室间隔、前壁、侧壁及后壁4个节段,分别测量糖尿病和正常人平均组织整合后散射积分(IB)和及其周期性变化曲线的形态和幅度(CVIB)。结果:正常组心肌组织整合后散射积分周期性变化曲线为随心动周期呈周期性变化的正弦彼,舒张末期为最高值,收缩末期为最低值,形态较规则。糖尿病组心肌组织整合后散射积分周期性变化曲线也为随心动周期呈周期性变化的正弦波,但振幅较正常人小。病例组所有节段的CVIB均低于正常人(P<0.05),而所有节段的IB均高于正常人(P<0.05)。结论:糖尿病患者心肌所有节段心肌组织密度均高于正常人,而心肌组织密度在心动周期中的变化均低于正常人。
Objective: The purpose of this study is to evaluate the characteristics and the value of cardiac myopathy in diabetic patients by using the new technology of myocardial backscatter quantification. Methods: The study was clinically diagnosed as 20 cases of type 2 diabetes. Another 20 cases of normal control group. Each section was divided into four sections: interventricular septum, anterior wall, lateral wall and posterior wall. The shape and amplitude of IB and CVIB were measured respectively after diabetes mellitus and normal people were integrated. Results: The cyclical variation curve of the integration score of myocardial tissue in the normal group was the sinusoidal cycle with periodic changes in the cardiac cycle, the highest in the end-diastole and the lowest in the end-systole. The curve of the cyclical changes in the integration score of myocardial tissue after diabetes mellitus was also a sine wave with cyclical changes in cardiac cycle, but the amplitude was smaller than that in normal people. The CVIB of all segments in the case group was lower than that of the normal people (P <0.05), while the IB of all the segments was higher than that of the normal people (P <0.05). Conclusion: The myocardial tissue density in all segments of myocardium of diabetic patients is higher than that in normal subjects, while the change of myocardial tissue density in cardiac cycles is lower than that in normal subjects.
