目的:研究乳腺癌组织中Foxp3+细胞浸润和Foxp3蛋白的表达及其临床意义。方法:160例乳腺癌以及22例癌旁正常乳腺组织取自上海市黄浦区中心医院(2005年1-12月),免疫组化检测乳腺癌组织和癌旁组织中Foxp3+淋巴细胞浸润和Foxp3蛋白的表达,分析它们与乳腺癌患者病理特征以及ER、PR、P53、Bcl-2、HER-2表达间的关系。结果:乳腺癌间质Foxp3+细胞浸润和实质Foxp3蛋白表达阳性率均高于癌旁组织(50.6%vs 13.6%,P<0.05;38.1%vs 4.5%,P<0.05);乳腺癌Foxp3+细胞浸润和Foxp3蛋白表达间无相关性(P>0.05)。乳腺癌间质Foxp3+细胞浸润与乳腺癌淋巴结转移、组织学分级、P53过表达呈正相关(P<0.05);癌实质Foxp3蛋白表达与肿瘤淋巴结转移、组织学分级、pTNM和P53过表达呈正相关(P<0.05);但两者与ER、PR、HER-2、Bcl-2过表达均无关(P>0.05)。单因素分析提示,乳腺癌中Foxp3表达和Foxp3+细胞浸润都是影响5年生存率的因素;多因素分析结果表明,Foxp3的表达不是影响乳腺癌5年生存率的独立预后指标。结论:乳腺癌组织高表达Foxp3蛋白可作为一个潜在的乳腺癌生物标记物,但不是一个独立的预后指标。
Objective: To investigate the expression of Foxp3 + infiltration and Foxp3 protein in breast cancer and its clinical significance. Methods: 160 cases of breast cancer and 22 cases of adjacent normal breast tissue were taken from Shanghai Huangpu District Central Hospital (January-December 2005). Immunohistochemistry was used to detect the infiltration of Foxp3 + lymphocytes and Foxp3 protein in breast cancer tissues and adjacent tissues The expression of ER, PR, P53, Bcl-2 and HER-2 in breast cancer patients were analyzed. Results: The positive rate of Foxp3 + cell infiltration and real Foxp3 expression in breast cancer was higher than that in paracancer tissues (50.6% vs 13.6%, P <0.05; 38.1% vs 4.5%, P <0.05) There was no correlation between Foxp3 protein expression (P> 0.05). The infiltration of Foxp3 + in breast cancer stroma was positively correlated with lymph node metastasis, histological grade and P53 overexpression in breast cancer (P <0.05). Foxp3 protein expression was positively correlated with tumor lymph node metastasis, histological grade, pTNM and P53 overexpression P <0.05). However, both of them had no correlation with ER, PR, HER-2 and Bcl-2 overexpression (P> 0.05). Univariate analysis showed that Foxp3 expression and Foxp3 + cell infiltration in breast cancer were all the factors affecting the 5-year survival rate. Multivariate analysis showed that Foxp3 expression was not an independent prognostic factor of breast cancer 5-year survival rate. Conclusion: High expression of Foxp3 protein in breast cancer tissue may serve as a potential biomarker of breast cancer, but it is not an independent prognostic indicator.
