目的探讨体质量指数(BMI)与臂踝动脉脉搏波传导速度(baPWV)的关系。方法以参加2010-2011(2010)、2012-2013(2012)、2014-2015(2014)年健康体检且参加baPWV检测的开滦集团职工22 681人为研究对象,排除了身高、体质量资料缺失者3009人,最终纳入统计分析的观察对象共计19 672人。按BMI将观察对象分成3组:正常组(BMI<24kg/m~2)、超重组(BMI 24~<28kg/m2)和肥胖组(BMI≥28kg/m~2),采用多因素Logistic回归模型分析不同BMI分组及BMI每增加一个标准差对baPWV≥14 m/s的影响。结果不同BMI分组间baPWV≥14m/s的检出率分别为:48.4%、60.8%、64.0%;多因素Logistic回归分析显示,在校正了年龄、性别、是否吸烟、饮酒、体育锻炼后,与正常组相比,baPWV≥14m/s的发生风险随BMI增加而呈上升趋势,然而进一步校正了平均动脉压(MAP)、心率、是否高血压、糖尿病、血脂异常、是否使用降压药、降糖药、调脂药等混杂因素后,超重和肥胖组发生baPWV≥14m/s的OR值(95%CI)分别为0.96(0.88~1.04)、0.85(0.76~0.96);另外,BMI每增加一个标准差发生baPWV≥14m/s的OR值(95%CI)为0.94(0.90~0.98)。结论在一般人群中BMI不是与baPWV关联良好的指标。
Objective To explore the relationship between body mass index (BMI) and brachial-ankle pulse wave velocity (baPWV). Methods A total of 22 681 Kailuan Group workers participated in the physical examination in 2010-2011 (2010), 2012-2013 (2012) and 2014-2015 (2014) and participated in baPWV testing were excluded from the study. In addition, A standard deviation of baPWV ≥ 14m / s OR value (95% CI) was 0.94 (0.90 ~ 0.98). Conclusions BMI is not a good indicator of association with baPWV in the general population.
