目的评价~(18)氟-胸腺嘧啶核苷(~(18)F-FLT)正电子发射体层(PET)-CT 显像对胸部肿瘤定性诊断的价值。方法对17例做了~(18)氟-脱氧葡萄糖(~(18)F-FDG)PET-CT 检查,但定性诊断困难的患者,在第2~3天进行了~(18)F-FLTPET-CT 显像。分析病变在两种不同显像剂 PET-CT 上的表现,分别测量二者的最大标准摄取值(maxSUV)。结果 8例恶性病变(5例肺癌、1例纵隔淋巴瘤、1例胸椎恶性肿瘤、1例胸椎转移瘤),其中7例见 FLT 异常摄取,5例肺癌平均 maxSUV为4.2(鳞癌2例、腺癌2例、肺泡癌1例);9例良性病变(5例肺结核、1例肺炎、3例纵隔淋巴结结核)无或轻度摄取 FLT,其中6例肺内良性病变平均 maxSUV 为1.6;9例良性病变中8例见 FDG 异常浓聚,其中6例肺内良性病灶平均 maxSUV 为3.9,3例纵隔淋巴结结核也见 FDG 明显摄取,平均 maxSUV 为11.0。11例肺内病灶 FDG 和 FLT 显像的敏感性、特异性、准确性分别为100.0%(5/5)、16.7%(1/6)、54.5%(6/11)和80.0%(4/5)、66.7%(4/6)、72.7%(8/11)。结论胸部肿瘤~(18)F-FLT 显像的特异性较高,在~(18)F-FDG 显像阳性,难以确定病变性质时,FLT 可以作为 FDG 的有益补充,二者联合显像有助于提高胸部肿瘤诊断的准确性。
Objective To evaluate the value of ~ (18) fluorodeoxythymidine (~ (18) F-FLT) positron emission tomography (PET-CT) in the qualitative diagnosis of chest tumor. Methods 18 patients with ~ (18) fluorodeoxyglucose (~ (18) F-FDG) PET-CT were examined in 17 cases. However, in patients with poor diagnosis, ~ (18) F-FLTPET -CT imaging. The lesions were evaluated on two different imaging agents, PET-CT, and the maximum standard uptake (maxSUV) of each was measured. Results 8 cases of malignant lesions (5 cases of lung cancer, 1 case of mediastinal lymphoma, 1 case of thoracic malignant tumor and 1 case of thoracic metastatic tumor) were found in 7 cases, and the average maxSUV in 5 cases of lung cancer was 4.2 (squamous cell carcinoma in 2 cases, 2 cases of adenocarcinoma and 1 case of alveolar carcinoma); 9 cases of benign lesions (5 cases of pulmonary tuberculosis, 1 case of pneumonia and 3 cases of mediastinal lymph node tuberculosis) had no or mild uptake of FLT, of which 6 cases had maxSUV of 1.6 8 cases of benign lesions, abnormal accumulation of FDG, 6 cases of benign lung lesions on average maxSUV was 3.9,3 cases of mediastinal lymph node tuberculosis also see FDG apparent uptake, the average maxSUV was 11.0.11 cases of lung lesions FDG and FLT imaging The sensitivity, specificity and accuracy were 100.0% (5/5), 16.7% (1/6), 54.5% (6/11), 80.0% (4/5) and 66.7% (4/6) , 72.7% (8/11). Conclusions The specificity of ~ (18) F-FLT imaging in chest tumor is high. FLT can be used as a good supplement for FDG in the positive of ~ (18) F-FDG imaging and difficult to determine the nature of the lesion. Help to improve the accuracy of chest tumor diagnosis.
