目的探讨血清半胱氨酸蛋白酶抑制剂C(胱抑素C,Cys-C)水平与帕金森病患者(Parkinson’s disease,PD)的相关性及临床意义。方法收集70例PD患者为观察组(PD组),30例健康体检者为对照组,根据Hoehn-Yahr(修正)分级量表(H&Y分期)将PD组分为3个亚组:早期组25例、中期组24例和晚期组21例。应用免疫比浊法检测血清Cys-C水平,比较PD组与对照组血清Cys-C水平的差异及PD组各亚组间血清Cys-C水平的差异;同时分析血清Cys-C与H&Y分期及帕金森病评定量表(UPDRS)运动评分的相关性。结果 PD组患者血清Cys-C水平明显高于对照组,差异具有统计学意义(t=3.61,P<0.01);晚期组血清Cys-C水平明显高于中期组及早期组,差异具有统计学意义(P<0.05,P<0.01);中期组血清Cys-C水平明显高于早期组及对照组,差异具有统计学意义(P<0.01,P<0.01);早期组血清Cys-C水平高于对照组,但差异无统计学意义(t=1.31,P=0.12)。PD组血清Cys-C水平与H&Y分期(r=0.67,P<0.01)、UPDRSⅡ(r=0.64,P<0.01)及UPDRSⅢ(r=0.59,P<0.01)均呈正相关。结论 PD患者血清Cys-C水平升高,且血清Cys-C水平与PD的病程进展相关,可反应病情的严重程度,在PD的进展中起一定作用。
Objective To investigate the correlation between serum cystatin C (Cys-C) and Parkinson’s disease (PD) and its clinical significance. Methods Seventy PD patients were selected as observation group (PD group) and 30 healthy subjects as control group. PD groups were divided into three subgroups according to Hoehn-Yahr (H & Y) grading scale: early group 25 Cases, 24 cases of mid-term group and 21 cases of advanced group. Serum levels of Cys-C were measured by immunoturbidimetry. The difference of serum Cys-C level between PD group and control group and Cys-C level among different subgroups of PD group were compared. Serum levels of Cys-C, H & Y, Correlation of Parkinson’s Disease Rating Scale (UPDRS) Motor Scores. Results The level of serum Cys-C in PD group was significantly higher than that in control group (t = 3.61, P <0.01). The level of serum Cys-C in advanced stage group was significantly higher than that in middle stage and early stage group (P <0.05, P <0.01). The level of serum Cys-C in the interim group was significantly higher than that in the early group and the control group (P <0.01, P <0.01) In the control group, but the difference was not statistically significant (t = 1.31, P = 0.12). Serum levels of Cys-C in PD group were positively correlated with H & Y stage (r = 0.67, P <0.01), UPDRSⅡ (r = 0.64, P <0.01) and UPDRSⅢ (r = 0.59, P <0.01). Conclusion The serum level of Cys-C is elevated in patients with PD, and serum Cys-C level correlates with the progression of PD, which may reflect the severity of the disease and play a role in the progression of PD.
