【目的】探讨金属硫蛋白(MT)和组织蛋白酶B(CatB)表达与结直肠癌淋巴结和肝转移的关系。【方法】采用免疫组化方法检测68例结直肠癌原发灶、正常结直肠黏膜、转移淋巴结和肝转移灶组织MT、CatB表达。对比分析MT、CatB表达与临床分期和病理类型的关系。【结果】在68例结直肠癌原发灶中MT、CatB表达阳性率分别为52.9%和47.1%;在正常肠黏膜中表达阳性率分别为26.5%、19.1%;结直肠癌原发灶中MT、CatB表达阳性率均高于正常肠黏膜组织(χ2=9.9512、11.9893,P<0.01)。按TNM分期,Ⅲ、Ⅳ期原发灶MT、CatB表达阳性率高于Ⅰ、Ⅱ期(χ2=9.9498、10.0192,P<0.01)。低分化腺癌MT、CatB表达阳性率高于高分化和中分化腺癌(χ2=8.8788、9.7183,P<0.01)。在34例转移淋巴结中MT和CatB共表达阳性率为47.1%;在16例肝转移组织中,MT和CatB共表达阳性率为50.0%。转移淋巴结和肝转移灶中MT和CatB共表达阳性率高于无转移的结直肠癌原发灶组织(χ2=5.7521、5.0794,P<0.05)。【结论】MT、CatB增强表达与结直肠癌淋巴结和肝转移有关,检测结直肠癌组织中MT、CatB表达可做为评价结直肠癌患者预后的重要指标。
【Objective】 To investigate the relationship between the expression of metallothionein (MT) and cathepsin B (CatB) and lymph node and liver metastasis of colorectal cancer. 【Methods】 The expressions of MT and CatB in 68 primary colorectal cancers, normal colorectal mucosa, metastatic lymph nodes and liver metastases were detected by immunohistochemistry. Comparative analysis of MT, CatB expression and clinical stage and pathological type of relationship. 【Results】 The positive rates of MT and CatB in primary colorectal cancer were 52.9% and 47.1%, respectively. The positive rates in normal intestinal mucosa were 26.5% and 19.1%, respectively. The positive rates of MT and CatB expression were higher than normal intestinal mucosa tissues (χ2=9.9512, 11.9893, P<0.01). According to TNM staging, the positive rates of MT and CatB expression in primary lesions of stage III and IV were higher than those of stages I and II (χ2=9.9498, 10.0192, P<0.01). The positive rates of MT and CatB expression in poorly differentiated adenocarcinoma were higher than those in well-differentiated and moderately differentiated adenocarcinoma (χ2=8.8788, 9.7183, P<0.01). The positive rate of MT and CatB co-expression was 47.1% in 34 metastatic lymph nodes, and the positive rate of MT and CatB co-expression was 50.0% in 16 liver metastases. The positive rates of MT and CatB co-expression in metastatic lymph nodes and liver metastases were higher than those in primary metastases (无2=5.7521, 5.0794, P<0.05). [Conclusion] The enhanced expression of MT and CatB is associated with lymph node and liver metastasis in colorectal cancer. Detecting the expression of MT and CatB in colorectal cancer can be used as an important index to evaluate the prognosis of colorectal cancer patients.
