目的测定血清胱抑素C(Cys-C)在糖尿病肾病患者中的水平,探讨血清胱抑素C是否是一种反映肾小球滤过率变化的理想的内源性标志物。方法 125例糖尿病患者按白蛋白排泄率(UAER)分为正常蛋白尿组39例(UAER<20μg/min,A组)、微量蛋白尿组44例(UAER20~199μg/min,B组)和大量蛋白尿组42例(UAER≥200μg/min,C组)。测定各组UAER、血清肌酐(Scr)、尿素氮(BUN)、肌酐清除率(Ccr)、Cys-C。结果果随着UAER的增加,Scr、BUN、Cys-C逐渐升高,而Ccr呈下降趋势;UAER、Scr、BUN、Cys-c与Ccr呈负相关。但Cys-C与Ccr相关性最强(r=-0.79,P=O.020)。结论论血清胱抑素C是一种反映早期糖尿病肾病肾小球滤过率变化的理想的标志物,相关性优于UAER、Scr和BUN。
Objective To determine the level of serum cystatin C (Cys-C) in patients with diabetic nephropathy and to explore whether serum cystatin C is an ideal endogenous marker reflecting the change of glomerular filtration rate. Methods 125 cases of diabetic patients were divided into normal proteinuria group (UAER <20μg / min, group A), microalbuminuria group 44 cases (UAER20 ~ 199μg / min, group B) 42 cases of proteinuria (UAER ≥ 200μg / min, C group). UAER, Scr, BUN, Ccr and Cys-C in each group were determined. Results With the increase of UAER, Scr, BUN and Cys-C increased gradually, while Ccr decreased. UAER, Scr, BUN and Cys-c had negative correlation with Ccr. However, Cys-C had the strongest correlation with Ccr (r = -0.79, P = 0.020). Conclusions Serum cystatin C is an ideal marker of glomerular filtration rate in early diabetic nephropathy. The correlation is better than that of UAER, Scr and BUN.
