目的三维斑点追踪技术(3D-STE)评价缺铁性贫血(IDA)患者的左心室(LV)心肌收缩功能。方法 65例IDA患者按照血红蛋白(Hb)的浓度分为两组:B组Hb>90g/L,C组Hb 60~90g/L。同时收集30例健康人作为对照组A组。对3组进行常规超声心动图检查,三维超声测量舒张末期左室容积(LVEDV)、收缩末期左室容积(LVESV)、左室射血分数(LVEF),并计算左室质量指数(LVMI),3D-STE分析左心室整体纵向应变(GLS)、整体面积应变(GAS)、整体径向应变(GRS)和整体周向应变(GCS)。结果 C组与B、A组间比较,LVEDV、LVESV和LVMI均升高,差异具有统计学意义(P均<0.05);GRS、GCS、GLS和GAS均减低,差异有统计学意义(P均<0.05)。结论 3D-STE能够有效地评估缺铁性贫血患者左心室心肌收缩功能的改变。
Objective To evaluate left ventricular (LV) myocardial systolic function in patients with iron deficiency anemia (IDA) by 3D-STEPS. Methods 65 patients with IDA were divided into two groups according to the concentration of hemoglobin (Hb): Hb> 90g / L in group B and 60 ~ 90g / L in group C. At the same time, 30 healthy people were collected as control group A Conventional echocardiography, left ventricular end-diastolic volume (LVEDV), left ventricular end-systolic volume (LVESV) and left ventricular ejection fraction (LVEF) were measured in three groups, and left ventricular mass index (LVMI) Left ventricular total longitudinal strain (GLS), global area strain (GAS), global radial strain (GRS) and global circumferential strain (GCS) were analyzed by 3D-STE. Results The LVEDV, LVESV and LVMI in group C were significantly higher than those in group B and group A (all P <0.05), while the levels of GRS, GCS, GLS and GAS were all decreased <0.05). Conclusion 3D-STE can effectively assess the changes of left ventricular systolic function in patients with iron deficiency anemia.
