目的探讨呼吸内科危重症哮喘的诊断及治疗效果。方法选取2014年1月至2014年12月期间遂平县人民医院收治的内科危重症哮喘患者50例,以药物糖皮质激素、β受体兴奋剂与茶碱类药物对患者进行治疗,同时辅以机械通气法对患者进行治疗,对患者的治疗效果进行分析。结果治疗后患者病症获得了一定的改善,主要体现如下:1心率:治疗前心率为(146±24)次/min,治疗后心率为(87±13)次/min;2呼吸频率:治疗前呼吸频率为(38±8)次/min,治疗后呼吸频率为(16±5)次/min;3pH值:治疗前PH值为(7.1±0.5),治疗后PH值为(7.4±0.4);4 PaO_2:治疗前PaO_2为(51.6±14.3)mm Hg,治疗后PaO_2为(92.8±15.2)mm Hg;5PaCO_2:治疗前PaCO_2为(69.2±8.3)mm Hg;治疗后PaCO_2为(36.3±7.0)mm Hg。治疗前后上述指标均存在统计学差异(P<0.05)。结论对于呼吸内科危重症哮喘患者以糖皮质激素、β受体兴奋剂、茶碱类药物及机械通气法进行治疗可取得较好的临床效果,值得广泛推广。
Objective To investigate the diagnosis and treatment of critically ill asthma in respiratory medicine. Methods From January 2014 to December 2014, 50 patients with medical critical asthma admitted to Suiping People’s Hospital were enrolled. Patients were treated with glucocorticoid, β-agonist and theophylline, The patients were treated by mechanical ventilation method, and the treatment effect of the patients was analyzed. Results After treatment, the patient’s condition got some improvement. The main manifestations were as follows: 1 Heart rate: The heart rate before treatment was (146 ± 24) times / min and the heart rate after treatment was (87 ± 13) times / min; The respiratory rate was (38 ± 8) times / min and the respiratory rate was (16 ± 5) times / min after treatment. The value of 3pH was (7.1 ± 0.5) before treatment and (7.4 ± 0.4) ; PaO_2 was (51.6 ± 14.3) mm Hg before treatment; PaO_2 was (92.8 ± 15.2) mm Hg after treatment; PaCO_2 was (69.2 ± 8.3) mm Hg before treatment; PaCO_2 was (36.3 ± 7.0) ) mm Hg. Before and after treatment, there was a statistically significant difference (P <0.05). Conclusion The treatment of glucocorticoids, β-receptor agonists, theophyllines and mechanical ventilation in patients with severe respiratory asthma may achieve better clinical results and should be widely promoted.
