目的探讨胸水miRNA-21和miRNA-155含量检测的临床意义。方法分别从43例恶性胸水患者和27例良性胸水患者胸水中提取miRNA,采用荧光定量PCR技术检测miRNA-21和miRNA-155在良性和恶性胸水中表达量,常规细胞学方法检测胸水脱落细胞。应用受试者工作特征曲线(ROC)分析方法评价其诊断价值。结果恶性胸水组miRNA-21和miRNA-155的表达量明显高于良性胸水组(P<0.05),胸水脱落细胞学检查肿瘤细胞阳性组miRNA-21和miRNA-155的表达量明显高于阴性组。胸水中miRNA-21表达量对肺癌的诊断价值:当特异性为100%时,诊断的敏感性为67.4%,ROC曲线下面积0.891。胸水中miRNA-155表达量对肺癌的诊断价值:当特异性为100%时,诊断的敏感性为65.1%,ROC曲线下面积0.848。结论检测胸水miRNA-21和miRNA-155的表达对诊断恶性胸水具有较高的特异性和敏感性。
Objective To investigate the clinical significance of detection of pleural fluid miRNA-21 and miRNA-155 content. Methods miRNAs were extracted from the pleural effusion of 43 patients with malignant pleural effusion and 27 patients with benign pleural effusion respectively. The expression of miRNA-21 and miRNA-155 in pleural effusions of benign and malignant pleural effusion was detected by real-time fluorescence quantitative PCR, and the exfoliated cells of pleural effusion were detected by routine cytology. The receiver operating characteristic curve (ROC) analysis method was used to evaluate the diagnostic value. Results The expression of miRNA-21 and miRNA-155 in malignant pleural effusion group was significantly higher than that in benign pleural effusion group (P <0.05). The expression levels of miRNA-21 and miRNA-155 in pleural effusion cytology were significantly higher than those in negative group . The diagnostic value of miRNA-21 expression in pleural effusion for lung cancer: When the specificity was 100%, the diagnostic sensitivity was 67.4% and the area under the ROC curve was 0.891. The diagnostic value of miRNA-155 expression in pleural effusion for lung cancer: When the specificity was 100%, the diagnostic sensitivity was 65.1% and the area under the ROC curve was 0.848. Conclusion Detection of pleural fluid miRNA-21 and miRNA-155 expression of high specificity and sensitivity for the diagnosis of malignant pleural effusion.
