目的比较18F-FDG PET-CT和99Tcm-MDP全身骨显像对骨转移瘤的诊断价值。方法经病理学证实为恶性肿瘤且临床怀疑合并骨转移的患者32例,均行全身18F-FDG PET-CT扫描和99Tcm-MDP全身骨显像(检查间隔时间在2周之内)。比较相同扫描野内两者对骨转移病灶检出的敏感性、特异性及准确性。结果PET-CT对骨转移瘤诊断的敏感性、特异性及准确性分别为94.9%、91.7%和94.1%,骨显像分别为96.2%、54.2%和86.3%。18F-FDGPET-CT对骨转移瘤的诊断特异性高于99Tcm-MDP骨显像,敏感性和准确性二者无显著差异。结论18F-FDG PET-CT与99Tcm-MDP骨显像相比,对骨转移瘤的诊断特异性较高,对肿瘤良恶性的鉴别优于骨显像。
Objective To compare the diagnostic value of 18F-FDG PET-CT and 99Tcm-MDP whole body bone imaging in bone metastases. Methods Thirty-two patients with pathologically confirmed malignant tumor and suspected bone metastasis were examined with whole-body 18F-FDG PET-CT scan and 99Tcm-MDP whole body bone scintigraphy (examination interval was within 2 weeks). To compare the sensitivity, specificity and accuracy of detecting both bone metastases in the same scan field. Results The sensitivity, specificity and accuracy of PET-CT in the diagnosis of bone metastases were 94.9%, 91.7% and 94.1%, respectively. The bone imaging was 96.2%, 54.2% and 86.3% respectively. The diagnostic specificity of 18F-FDG PET-CT for bone metastases was higher than 99Tcm-MDP bone scintigraphy with no significant difference in sensitivity and accuracy. Conclusion Compared with 99Tcm-MDP bone imaging, 18F-FDG PET-CT is more specific for the diagnosis of bone metastases and is superior to bone imaging in differentiating benign and malignant tumors.
