目的:分析长程视频脑电图(video electroencephalogram,VEEG)监测对睡眠相关性癫痫的诊断价值。方法:对2013年7月—2014年11月收集的119例睡眠相关性癫痫患者的VEEG及常规脑电图(routine electroencephalogram,REEG)进行回顾性分析。结果:本组患者REEG监测的总异常检出率为33.61%,VEEG监测的总异常检出率为98.32%,两种方法比较差异有统计学意义(χ2=109.83,P<0.001)。REEG监测的痫样放电检出率为18.49%,VEEG监测的痫样放电检出率为85.71%,两种方法比较差异有统计学意义(χ2=107.75,P<0.001)。REEG监测本组患者无临床事件发生,VEEG监测中有42例患者记录到临床事件。结论:VEEG监测显著提高痫样放电的检出率,更好地监测睡眠中的痫样放电、发作期脑电及发作期临床表现,对睡眠相关性癫痫的诊断及明确发作类型具有重要价值。
Objective: To analyze the diagnostic value of long-range video electroencephalogram (VEEG) for sleep-related epilepsy. Methods: A retrospective analysis was performed on VEEG and routine electroencephalogram (REEG) in 119 sleep-related epilepsy patients collected from July 2013 to November 2014. Results: The total abnormal detection rate of REEG in this group was 33.61% and the total abnormal detection rate of VEEG was 98.32%. The difference between the two methods was statistically significant (χ2 = 109.83, P <0.001). The detection rate of epileptiform discharge by REEG was 18.49%, and the detection rate of epileptiform discharge by VEEG was 85.71%. The difference between the two methods was statistically significant (χ2 = 107.75, P <0.001). No adverse events were detected by REEG in this group of patients, and 42 patients in VEEG monitoring recorded clinical events. Conclusion: VEEG monitoring significantly improves the detection rate of epileptiform discharges, and better monitors epileptiform discharge during sleep, EEG and the clinical manifestations during the attack, which is of great value in the diagnosis and definite type of sleep-related epilepsy.
