患儿,男,5个月.因咳嗽气喘20＋d,加重10h于2012年12月22日入院.患儿中度昏迷,全身湿冷,遍布花斑,颜面口唇紫绀,呼吸急促,肌张力稍高,听诊双肺哮鸣音及湿啰音,诊断为呼吸衰竭、心力衰竭.经吸氧、吸痰后缺氧及气道梗阻无缓解,血氧饱和度（SpO2）60%,予经口气管插管,呼吸机辅助通气,模式同步间歇指令通气（SIMV）.通气5min后查动脉血气分析示：pH 6.65,二氧化碳分压（PCO2）＞130mm Hg,氧分压（PO2）38mm Hg,乳酸（Lac）16.17mmol/L,予纠正高乳酸中毒,
