目的观察活血止痛散联合西药治疗早期糖尿病足的临床疗效。方法将120例早期糖尿病足患者随机分成治疗组与对照组,每组60例。对照组常规西药治疗,治疗组在对照组治疗措施基础上加服活血止痛散。两组疗程均为20天,观察临床疗效,比较血糖水平、足部踝肱比及趾肱比、深浅感觉、运动及感觉神经传导速度的变化情况。结果 (1)治疗组、对照组临床总有效率分别为80.00%、55.00%;组间临床疗效比较,治疗组明显优于对照组(P<0.05)。(2)组间治疗后比较,空腹血糖、餐后2 h血糖水平差异有统计学意义,治疗组明显低于对照组(P<0.05)。(3)组间治疗后比较,踝肱比、趾肱比水平差异有统计学意义,治疗组明显较对照组升高(P<0.05)。(4)组间治疗后比较,浅感觉、深感觉水平差异有统计学意义,治疗组感觉功能提高明显优于对照组(P<0.05)。(5)组间治疗后比较,腓总神经、正中神经传导速度差异有统计学意义,治疗组神经传导速度明显快于对照组(P<0.05)。结论活血止痛散联合西药治疗早期糖尿病足,可明显缓解患者的临床症状,降低血糖水平,改善足部的深浅感觉及神经功能,从而有效控制本病的病情发展。
Objective To observe the clinical efficacy of Huoxue Zhitong San combined with western medicine in treating early diabetic foot. Methods 120 patients with early diabetic foot were randomly divided into treatment group and control group, 60 cases in each group. The control group routine western medicine treatment group in the control group based on the treatment measures plus service Huoxuezhitong San. The two groups were treated for 20 days. The clinical curative effect was observed. The changes of blood glucose level, ankle-brachial ratio, toe-brachii ratio, shady sensation, motor and sensory nerve conduction velocity were compared. Results (1) The total clinical effective rates of the treatment group and the control group were 80.00% and 55.00% respectively. The clinical efficacy of the treatment group was significantly better than that of the control group (P <0.05). (2) After treatment, fasting blood glucose and postprandial 2h blood glucose levels were significantly different, the treatment group was significantly lower than the control group (P <0.05). (3) After treatment, the ankle-brachial ratio and toe-brachial ratio had statistical significance, the treatment group was significantly higher than the control group (P <0.05). (4) After treatment, there was significant difference between light sensation and deep sensory sensation. The improvement of sensory function in treatment group was obviously better than that in control group (P <0.05). (5) After treatment, there were significant differences in the velocity of the common peroneal nerve and the median nerve conduction between the two groups. The nerve conduction velocity in the treatment group was significantly faster than that in the control group (P <0.05). Conclusions Huoxue Zhitong Powder combined with Western medicine in the treatment of early diabetic foot can obviously relieve the clinical symptoms, reduce the blood sugar level, improve the sensation of foot and nerve function, and thus effectively control the disease progression.
