目的探讨口腔癌患者术后早期外周血免疫功能指标的变化。方法选取2014年6月至2016年6月间山东中医药大学第二附属医院收治的70例口腔癌患者,分别于手术前后抽取患者外周静脉血,对外周血中免疫功能指标CD3~+、CD3~+CD4~+、CD3~+CD8~+、CD4~+/CD8~+、T+B+NK细胞、CD19+和CD16+/CD56+进行检测,另选取同期我院体检的70例健康人员外周静脉血免疫功能指标检测结果作为正常参考值。结果口腔癌患者术后1周的免疫功能指标与正常值比较:CD3~+CD8~+升高,CD4~+/CD8~+降低,差异均有统计学意义(均P<0.05)。与术前1周比较,口腔癌患者术后1周CD3~+CD8~+降低,术后2周CD3~+CD8~+升高,但仍低于术前1周水平,差异均有统计学意义(均P<0.05)。与术前1周比较,口腔癌患者术后1周和术后2周的CD4~+/CD8~+均升高,差异均有统计学意义(均P<0.05)。与术前1周比较,口腔癌患者术后2周T+B+NK细胞较低,差异有统计学意义(P<0.05)。口腔癌患者术后1周CD19+与术前1周比较升高,术后2周与术后1周比较较低,但仍高于术前1周,差异有统计学意义(P<0.05)。与术前1周比较。口腔癌患者术后1周和术后2周CD16+/CD56+均较低,差异均有统计学意义(均P<0.05)。结论口腔癌患者免疫功能受到一定程度的抑制,通过手术治疗能够有效缓解细胞免疫抑制作用。
Objective To investigate the changes of peripheral blood immune function in patients with oral cancer after operation. Methods Seventy patients with oral cancer who were admitted to the Second Affiliated Hospital of Shandong University of Traditional Chinese Medicine from June 2014 to June 2016 were enrolled in this study. Peripheral venous blood was collected before and after surgery. The levels of CD3 +, CD3 CD3 ~ + CD8 ~ +, CD4 ~ + / CD8 ~ +, T + B + NK cells, CD19 + and CD16 + / CD56 + were detected in peripheral blood of 70 healthy volunteers in our hospital during the same period. Immune function test results as a normal reference value. Results Compared with normal one week after operation, the immune function indexes of oral cancer patients were higher: CD3 ~ + CD8 ~ + and CD4 ~ + / CD8 ~ + decreased (all P <0.05). Compared with 1 week before operation, CD3 ~ + CD8 ~ + decreased 1 week after operation in patients with oral cancer, and CD3 ~ + CD8 + increased 2 weeks after operation, but still lower than 1 week before operation, the differences were statistically significant Significance (all P <0.05). Compared with one week before surgery, the levels of CD4 ~ + / CD8 ~ + in patients with oral cancer at 1 week and 2 weeks after operation were significantly increased (all P <0.05). Compared with the one week before operation, T + B + NK cells in oral cancer patients were lower at 2 weeks after operation, the difference was statistically significant (P <0.05). The level of CD19 + in patients with oral cancer at one week after operation was significantly higher than that at one week before operation, but lower at 2 weeks and 1 week after operation than the one week before operation. The difference was statistically significant (P <0.05). Compared with 1 week before surgery. The CD16 + / CD56 + levels in patients with oral cancer at 1 week and 2 weeks after operation were significantly lower (all P <0.05). Conclusion The immune function of oral cancer patients is inhibited to a certain extent. The surgical treatment can effectively relieve the cellular immunosuppression.
