目的 采用双功彩色多普勒血流显像和99mTc MDP乳房显像相结合评价乳腺良恶性肿瘤的鉴别诊断价值。方法 用CDFI检查 1 5 2例 1 5 8个乳腺肿块 ,定量分析彩色像素平均密度(MCPD)和血管数目 ,用PD取样计算Vmax、Vmin、Vmean及RI值 ;用99mTc MDP检测 32例乳腺肿块 ,计算对乳腺癌诊断的敏感性、特异性、准确性。结果 提示彩色像素平均密度 >0 0 6 1± 0 0 0 1 7,峰值血流速度 >2 3 5 5cm/s± 1 2 3,RI >0 71± 0 0 1 1 ,血管数目 >1 6 7± 0 0 6 5 ,99mTc MDP乳房显像阳性 ,考虑为乳腺癌 ,其敏感性 88%、特异性 82 %、准确性 89% ;99mTc MDP的敏感性 90 %、特异性80 %、准确性 87%。结论 CDFI、PD和 99mTc MDP联合运用对乳腺良恶性肿瘤的鉴别诊断有较高的临床价值。
Objective To evaluate the differential diagnosis value of dual benign color Doppler flow imaging and 99mTc MDP breast imaging in breast benign and malignant tumors. Methods A total of 152 breast masses were examined by CDFI. The mean color pixel density (MCPD) and the number of blood vessels were quantitatively analyzed. Vmax, Vmin, Vmean, and RI were calculated by PD sampling. Thirty-two breast masses were detected by 99mTc MDP. Calculate the sensitivity, specificity, and accuracy of breast cancer diagnosis. The results suggest that the average color pixel density> 0 0 61 ± 0 0 0 1 7. Peak blood flow velocity> 2 3 5 5cm/s ± 1 2 3, RI> 0 71 ± 0 0 1 1 and number of blood vessels> 1 6 7 ± 0 0 6 5 , 99mTc MDP positive breast imaging, considering sensitivity to breast cancer, 88% sensitivity, specificity 82%, accuracy 89%; 99mTc MDP sensitivity 90%, specificity 80%, accuracy 87 %. Conclusion The combined use of CDFI, PD and 99mTc MDP has a high clinical value in differential diagnosis of benign and malignant breast tumors.
