目的 :探讨动态血糖监测指导下胰岛素泵强化治疗在2型糖尿病患者中的临床疗效。方法 :将90例患者随机分为:A组采用动态血糖监测系统联合胰岛素泵治疗;B组采用指血血糖监测联合胰岛素泵治疗;C组采用指血血糖监测联合预混胰岛素治疗。每组30例。观察治疗2周后各组血糖水平、血糖达标天数、平均血糖漂移幅度、胰岛素用量及低血糖检出率等指标。结果:1治疗后,3组患者的空腹血糖及餐后2 h血糖均较治疗前显著下降,差异有统计学意义(P<0.01);A组及B组治疗后的餐后2 h血糖水平较C组显著下降,差异有统计学意义(P<0.05);各组间治疗后的空腹血糖水平无统计学差异(P>0.05);2A组和B组血糖达标天数、胰岛素用量显著少于C组,差异有统计学意义(P<0.05);A组的平均血糖漂移幅度显著低于B组和C组,差异有统计学意义(P<0.01);3A组低血糖的检出率明显高于B组(P<0.05)和C组(P<0.01),差异有统计学意义。结论:动态血糖监测联合胰岛素泵治疗方案可更有效地控制血糖,减少胰岛素用量、血糖达标天数及血糖漂移幅度,提高低血糖的检出率。
Objective: To investigate the clinical curative effect of insulin pump intensive therapy in type 2 diabetes patients under the guidance of dynamic blood glucose monitoring. Methods: 90 patients were randomly divided into: A group using dynamic glucose monitoring system combined with insulin pump treatment; B group using blood glucose monitoring with insulin pump treatment; C group using blood glucose monitoring with premixed insulin therapy. 30 cases in each group. Two weeks after the treatment, the blood sugar level, the number of days of reaching the blood glucose level, the average blood glucose level, the amount of insulin and the rate of hypoglycemia were observed. Results: After treatment, the fasting blood glucose and postprandial 2-h blood glucose in three groups were significantly decreased compared with those before treatment (P <0.01); the blood glucose levels at 2 h after treatment in groups A and B (P <0.05). There was no significant difference in the fasting blood glucose level between the two groups after treatment (P> 0.05). The blood glucose levels in the two groups were significantly lower than those in the two groups The difference was statistically significant (P <0.05); the average blood glucose drift in group A was significantly lower than that in group B and C (P <0.01); the detection rate of hypoglycemia in group 3A was significantly higher than that in group C Higher than B group (P <0.05) and C group (P <0.01), the difference was statistically significant. Conclusion: The combination of dynamic blood glucose monitoring and insulin pump can control blood sugar more effectively, reduce the amount of insulin, the number of days of blood sugar standard and the blood sugar drift, and increase the detection rate of hypoglycemia.
