目的 检测上海地区幽门螺杆菌 (Hp)感染患者中Hp菌株iceA、babA2的分布特征 ,探讨与Hp临床感染结局相关的菌株基因型。 方法 1 4 1株Hp菌株分离自 4 3例慢性胃炎 (CG)、4 7例十二指肠球部溃疡 (DU)、30例胃溃疡 (GU)和 2 1例非贲门部胃癌患者的胃镜活检标本。采用PCR方法检测Hp菌株的iceA、babA2、cagA和vacA基因型。 结果 1 4 1株Hp菌株中 ,iceA1、iceA2和babA2的总检出率分别为 74 .5 % (1 0 5 /1 4 1 )、1 5 .6 % (2 2 /1 4 1 )和 6 3.8% (90 /1 4 1 ) ,其中 2例 (1 .4 % )为iceA1、iceA2均阳性 ,1 6例 (1 1 .3% )为iceA1、iceA2均阴性。DU组的babA2检出率显著高于GU组 (74 .5 %比5 0 .0 % ,P =0 .0 2 8) ,DU组的cagA+ /babA2 + 检出率亦显著高于GU组 (70 .2 %比 4 6 .7% ,P =0 .0 39)。其余疾病组之间的babA2检出率差异无显著性。未发现不同临床疾病与iceA基因型的相关性。结论 上海地区Hp感染者的菌株基因型主要是iceA1 + /babA2 + ,babA2在DU和GU的发病机制中起不同作用。未发现iceA亚型与Hp临床感染结局有相关性
Objective To investigate the distribution of Hp strains iceA and babA2 in patients with H.pylori infection in Shanghai and to explore the genotypes of strains associated with Hp clinical infection. Methods One hundred and forty-one Hp strains were isolated from gastroscopy of 43 chronic gastritis (CG), 47 duodenal ulcer (DU), 30 gastric ulcer (GU) and 21 non-cardia gastric cancer Biopsy specimens. The iceA, babA2, cagA and vacA genotypes of Hp strains were detected by PCR. Results The total detection rates of iceA1, iceA2 and babA2 in 14 strains of Hp strains were 74.5% (105/141), 1.56% (2 2/1 141) and 6 3.8% (90/141), of which 2 cases (1.4%) were iceA1, iceA2 were positive, 16 cases (11.3%) were iceA1, iceA2 were negative. The detection rate of babA2 in DU group was significantly higher than that in GU group (74.5% vs 50.0%, P = 0.028). The detection rate of cagA + / babA2 + in DU group was also significantly higher than that in GU group 70.2% vs 46.6%, P = 0.039). There were no significant differences in the detection rates of babA2 between the other disease groups. No correlation between different clinical diseases and iceA genotypes was found. Conclusion The genotypes of Hp-infected patients in Shanghai are mainly iceA1 + / babA2 +, and babA2 plays different roles in the pathogenesis of DU and GU. No association was found between the iceA subtype and the clinical outcome of Hp
