目的探讨剖宫产术后未满6个月意外妊娠的影响因素及对策。方法采用回顾性分析方法对2010年2月至2014年10月在舟山市普陀区中医院行剖宫产手术产妇2 100例的临床资料进行分析,采用单因素和多因素Logistic回归分析剖宫产术后未满6个月意外妊娠影响因素。结果 2 100例剖宫产术产妇术后未满6个月意外妊娠有218例,发生率占10.38%。经单因素分析显示文化程度(x~2=41.466)、户籍(x~2=13.979)、侥幸心理(x~2=335.624)、避孕知识缺乏(x~2=484.951)、有无避孕措施(x~2=517.928)、丈夫文化程度(x~2=18.345)、避孕药(具)使用方法不当(x~2=5.906)是剖宫产术后未满6个月意外妊娠发生的影响因素(均P<0.05);经Logistic回归多因素分析显示文化程度(OR=4.876,95%CI:1.632~10.743)、户籍(OR=5.252,95%CI:1.726~18.121)、侥幸心理(OR=5.373,95%CI:2.466~15.542)、避孕知识缺乏(OR=0.446,95%CI:0.121~0.935)、有无避孕措施(OR=0.336,95%CI:0.126~0.749)是剖宫产术后未满6个月意外妊娠发生的独立影响因素(均P<0.01)。结论文化程度、户籍、侥幸心理、避孕知识缺乏、有无避孕措施是剖宫产术后未满6个月意外妊娠的独立影响因素,加强产后生殖健康教育是避免产后意外妊娠最有效的措施。
Objective To investigate the influencing factors and countermeasures of unplanned pregnancy in less than 6 months after cesarean section. Methods The clinical data of 21 100 maternal women undergoing cesarean section at Putuo District Hospital of Traditional Chinese Medicine in Zhoushan from February 2010 to October 2014 were analyzed retrospectively. Single factor and multivariate logistic regression analysis were performed on cesarean section The influencing factors of unplanned pregnancy after less than 6 months. Results 2 100 cases of cesarean section maternal less than 6 months pregnant 218 cases of unwanted pregnancy, the incidence of 10.38%. Univariate analysis showed that education level (x ~ 2 = 41.466), household registration (x ~ 2 = 13.979), fluke psychology (x ~ 2 = 335.624), lack of knowledge of contraception (x 2 = 517.928), husband’s educational level (x 2 = 18.345) and improper use of contraceptives (x 2 = 5.906) were the influencing factors of unplanned pregnancies less than 6 months after cesarean section (OR = 4.876,95% CI: 1.632-10.743), family register (OR = 5.252,95% CI: 1.726-18.121), chance of being lucky (OR = (OR = 0.336, 95% CI: 0.126 ~ 0.749) was a cesarean section (5.373,95% CI: 2.466-15.542), with a lack of knowledge about contraception (OR = 0.446, 95% CI: 0.121-0.935) The independent influencing factors of unplanned pregnancy occurred in less than 6 months (all P <0.01). Conclusion The educational level, household registration, luck and lack of knowledge of contraception, and the availability of contraceptive measures are the independent influencing factors of unplanned pregnancy within 6 months after cesarean section. Strengthening postpartum reproductive health education is the most effective measure to prevent postpartum unwanted pregnancies.
