腭裂是颌面部较常见的先天性发育畸形,其原发或继发畸形均会影响患者的口颌系统功能和身心健康。牵张成骨(DO)是矫治颌面部骨骼畸形和组织缺损的新型外科技术之一,利用其矫治先天性腭裂的原发或继发畸形的研究已取得一定的效果。本文对动物实验和临床应用中的术期选择、手术方式、牵张装置的设计、牵张时机、可能的并发症及其预后等方面进行综述,以诠释DO技术在关闭原发性腭裂裂隙、延长硬腭长度、矫治牙槽突裂、矫治继发性上颌骨发育不全中的研究和应用。
Cleft palate is a common congenital malformation in maxillofacial region. The primary or secondary deformity will affect the patient’s oral mandibular system function and physical and mental health. Distraction osteogenesis (DO) is one of the new surgical techniques for the treatment of skeletal deformities and tissue defects in maxillofacial region. The study of primary and secondary deformities in the treatment of congenital cleft palate has achieved some results. This review summarizes the aspects of operation choice, operation mode, design of distraction device, timing of distraction, possible complications and prognosis in animal experiments and clinical applications, in order to explain DO technology in the closure of primary cleft palate, Extend the length of the palate, correction of alveolar cleft, correction of secondary maxillary hypoplasia research and application.
