目的探索HLA-A、B等位基因对内蒙地区汉族儿童过敏性紫癜(AP)的遗传易感性。方法采用病例对照研究方法,引入PCR-SSO技术,在祖籍三代居住内蒙地区、无血缘关系、无与异族通婚史及其他免疫性疾病史和家族史的汉族人群中,选择儿童AP病例组50例和健康儿童对照组96例,作以HLA- A、B等位基因型别分析。基因频率比较在单因素四格表x~2或Fisher检验的基础上,作以多因素logistic回归分析。结果HLA-A※26、HLA-B※35和HLA-B※52基因频率分别是7%、11%和9%,分别与健康对照组的1%、4.7%和3.6%比较,差异有统计学意义(x~2分别是-、4.107和3.639,P分别是0.009、0.043和0.056)。经回归后,Wald分剐是4.783、4.205和4.293,P值分别是0.029、0.040和0.038,差异均有统计学意义(均P<0.05)。B分别是1.782、1.112和1.164,均B>0;OR分别是5.941、3.041和3.202,均OR>1,促进发病,其95%可信区间分别是1.203~29.331、1.050~8806和1.065~9627,其内均不包含1,与P意义相符,兼有统计学意义。EF分别是0.647、0.369和0.387,均EF>0。结论HLA-A※26、HLA-B※35和HLA -B※52等位基因可能是内蒙地区汉族儿童AP发病单体型中的3个遗传易感基因。
Objective To explore the genetic predisposition of HLA-A and B alleles to Henoch-Schonlein purpura (AP) in children in Inner Mongolia. Methods A case-control study was conducted in which 50 cases of children with AP were selected from the group of children with AP by PCR-SSO technique in the Han nationality who lived in Inner Mongolia for three generations and had no history of kinship, non-inter-ethnic marriage and other immunological diseases and family history. And healthy children control group of 96 cases for HLA- A, B allele type analysis. Gene frequency comparison in the single factor four grid table x ~ 2 or Fisher’s test, based on multivariate logistic regression analysis. Results The frequencies of HLA-A * 26, HLA-B * 35 and HLA-B * 52 genes were 7%, 11% and 9%, respectively, which were respectively compared with 1%, 4.7% and 3.6% of healthy controls Significance (x ~ 2 respectively -, 4.107 and 3.639, P respectively 0.009,0.043 and 0.056). After regression, the Wald scores were 4.783, 4.205 and 4.293, P values were 0.029, 0.040 and 0.038, respectively, with significant differences (all P <0.05). B were 1.782,1.112 and 1.164, respectively, B> 0; OR were 5.941,3.041 and 3.202, all OR> 1, to promote the disease, the 95% confidence interval were 1.203 ~ 29.331,1.050 ~ 8806 and 1.065 ~ 9627 , Which do not contain 1, consistent with the meaning of P, both statistically significant. EF were 0.647,0.369 and 0.387, all EF> 0. Conclusion The HLA-A * 26, HLA-B * 35 and HLA-B * 52 alleles may be the three susceptibility genes in the AP haplotypes in Han children in Inner Mongolia.
