本文报道我科1977年至1987年手术切除食管癌873例,术后切缘癌残留88例,发生率为10.1%。术后1、5、10年生存率为72.7%、15.9%、9.2%。分析认为食管癌术后切缘癌残留的主要原因是食管切除长度不足,特别是胸中段食管癌。因此,应当切除足够长度的食管,行食管一胃颈部吻合术和术前放疗,以减少切缘癌残留的发生。
This article reports 873 surgically resected esophageal cancers in our department from 1977 to 1987. There were 88 cases of residual margin after surgery, and the incidence was 10.1%. The 1-, 5-, and 10-year survival rates were 72.7%, 15.9%, and 9.2%. According to the analysis, the main reason for the resection of esophageal cancer after surgical resection is insufficient length of esophageal resection, especially in the mid-thoracic esophageal cancer. Therefore, a sufficient length of esophagus should be removed, an esophageal-gastric neck anastomosis, and preoperative radiotherapy should be performed in order to reduce the occurrence of marginal cancer remnants.
