目的探讨脑梗死患者昼夜血浆内皮素(ET)浓度变化与平均动脉压(MAP)及心率(HR)的关系。方法对48例急性脑梗死(ACI)病人入院后佩带24h血压、心率动态检测仪自动记录心率、血压 ,同时采用放射免疫测定法分别测定2h、8h、14h、20h血浆ET浓度。结果ACI者血浆ET、MAP及HR都有明显的夜间2h低谷 ,晨起后8h高峰的昼夜节律性变化 ,并且ACI者血浆ET及MAP24h均明显高于正常对照组(P<0.05~0.01) ,两者呈显著正相关性(P<0.05~0.01)。有高血压病史的ACI者血浆ET及MAP显著高于无高血压病史者(P<0.05~0.01)。结论脑梗死的发生、发展与血浆ET、MAP有密切关系 ,血浆ET及MAP增高可促进脑梗死的发生发展 ,反之亦然。三者相互影响促进脑梗塞的发展
Objective To investigate the relationship between diurnal plasma concentration of endothelin (ET) and mean arterial pressure (MAP) and heart rate (HR) in patients with cerebral infarction. Methods 48 patients with acute cerebral infarction (ACI) were admitted to hospital for 24 hours after they were admitted to hospital. Heart rate and blood pressure were recorded automatically by heart rate dynamic detector. Plasma ET concentrations were measured by radioimmunoassay at 2h, 8h, 14h and 20h respectively. Results The ET, MAP and HR in plasma of ACI patients were both significantly lower at 2h at night and at 8h after morning than that at 8h. ACI plasma ET and MAP24h were significantly higher than that of normal control group (P <0.05 ~ 0.01) There was a significant positive correlation between them (P <0.05 ~ 0.01). The plasma ET and MAP of ACI with history of hypertension were significantly higher than those without history of hypertension (P <0.05 ~ 0.01). Conclusions The occurrence and development of cerebral infarction are closely related to plasma ET and MAP. The increase of plasma ET and MAP can promote the development of cerebral infarction and vice versa. Interaction between the three to promote the development of cerebral infarction
