目的探讨p75NTR在胃癌组织中的表达及临床意义。方法选择2015年1月—2016年1月在解放军白求恩国际和平医院行手术切除的胃癌60例,将其胃癌组织标本作为研究对象,根据患者淋巴结是否转移及病灶浸润深度进行分组。统计比较淋巴结转移阴性组、淋巴结转移阳性组以及病灶浸润肌层组、病灶浸润全层组p75NTR检测情况,并对淋巴结转移阴性组和淋巴结转移阳性组治疗后生活质量进行比较。结果淋巴结转移阴性组p75NTR检出阳性率显著高于淋巴结转移阳性组,两组比较差异有统计学意义(P<0.05)。病灶浸润肌层组p75NTR检出阳性率显著高于病灶浸润全层组,两组比较差异亦具有统计学意义(P<0.05)。治疗后淋巴结转移阴性组日常生活能力评分、躯体功能评分、心理功能评分及社会功能评分均显著高于淋巴结转移阳性组,两组比较差异均具有统计学意义(P<0.05)。结论 p75NTR水平检测可提示胃癌组织淋巴结转移情况以及病灶浸润深度,p75NTR水平检测可作为预测胃癌患者预后情况的依据。
Objective To investigate the expression of p75NTR in gastric carcinoma and its clinical significance. Methods From January 2015 to January 2016, 60 cases of gastric cancer underwent surgical resection at the Bethune International Peace Hospital of the People’s Liberation Army. Their gastric cancer tissues were selected as the study group and were divided into groups according to their lymph node metastasis and depth of invasion. The levels of p75NTR in the lymph node metastasis-negative group, lymph node metastasis-positive group, focal infiltration myometrium group and the lesion-infiltrating myometrial group were statistically compared, and the quality of life was compared between the negative lymph node metastasis group and the positive lymph node metastasis group. Results The positive rate of p75NTR in lymph node negative group was significantly higher than that in lymph node positive group (P <0.05). The positive rate of p75NTR in infiltrating myometrium was significantly higher than that in the lesion-infiltrating full-thickness group, and the difference between the two groups was also statistically significant (P <0.05). The scores of daily living ability, physical function score, psychological function score and social function score of negative lymph node metastasis group after treatment were significantly higher than those of positive lymph node metastasis group. The difference between the two groups was statistically significant (P <0.05). Conclusion The detection of p75NTR can be used to predict the lymph node metastasis and depth of invasion in gastric cancer. The detection of p75NTR can be used as a basis for predicting the prognosis of gastric cancer.
