目的运用盆底实时三维超声技术观察29例我院行经闭孔阴道无张力尿道中段悬吊带(TVT-O)术后的吊带情况。方法对29例行TVT-O手术的患者于其术后3个月按临床疗效的差异进行分组及盆底实时三维超声检查,观察植入的悬吊带并测量相关参数,并动态观察患者在静息、Valsalva动作时吊带的活动和形态变化。结果被检患者中症状治愈者有23例,症状改善者有6例,两组患者在静息时吊带近端—膀胱颈距离、吊带近端—膀胱颈距离与尿道全长比值、吊带横截面宽度、吊带成角以及Valsalva动作时吊带中点与耻骨联合后下缘距离、吊带中点至尿道中点距离、吊带成角等参数均有差异(P<0.05)。结论盆底实时三维超声能观察TVT-O术后的吊带悬吊情况,为患者术后的疗效评估及处理提供有价值的影像学依据及指导意见。
Objective To observe the sling of 29 cases of transvaginal tension-free urethral middle sling (TVT-O) in our hospital with real-time three-dimensional pelvic floor ultrasound. Methods Twenty-nine patients undergoing TVT-O operation were divided into groups and real-time three-dimensional echocardiography according to the difference of clinical curative effect 3 months after operation. The implanted sling was observed and the related parameters were measured. Interest, Valsalva action when sling activity and morphological changes. Results In the seized patients, 23 cases were cured of symptoms, 6 cases were improved in symptoms. The distance between the proximal and the bladder neck at resting, the distance between the proximal and the bladder neck and the length of the urethra in both groups, The width, the angle of the sling, the distance between the midpoint of the sling and the lower edge of the pubic symphysis, the distance from the midpoint of the sling to the midpoint of the urethra, and the angle of the sling were all significantly different (P <0.05). Conclusion The pelvic floor real-time three-dimensional ultrasound can observe the sling suspension after TVT-O operation and provide valuable imaging basis and guidance for the postoperative evaluation and treatment of patients.
