目的 研究和探讨缺血性视神经病变 (ischemicopticneuropathy ,IONP)的眼血管和颈内动脉的血流动力学改变。 方法 应用彩色多普勒显像 (colorDopplerimaging ,CDI)技术对 30例缺血性视神经病变的眼血管和颈内动脉的血流进行检测 ,并与其健眼进行对照观察。结果 患眼与健眼比较 ,视网膜中央动脉阻力指数显著增大 ,P <0 0 0 1;视网膜中央动脉搏动指数和睫状后动脉阻力指数明显增大 ,P <0 0 1;睫状后动脉舒张末期血流速度和搏动指数增大 ,P <0 0 5 ;睫状后动脉的血流频谱峰值后移 ,峰钝变形呈现血管高阻力改变。同时 ,分析了IONP预后与颈内动脉血流异常的关系。结论 CDI技术能为临床IONP的诊断和治疗提供血流动力学依据。为临床研究视网膜和视神经病生理血液微循环提供有效信息。
Objective To study and investigate the hemodynamic changes of ocular and internal carotid arteries of ischemic optic neuropathy (IONP). Methods Color Doppler imaging (CDI) was used to detect the blood flow of ocular and internal carotid arteries in 30 patients with ischemic optic neuropathy and compared with healthy eye. Results Compared with healthy eye, the index of central retinal artery resistance increased significantly (P <0.01); the central retinal artery and ciliary artery resistance index increased significantly (P <0.01); the posterior ciliary artery The end diastolic blood flow velocity and pulsatility index increased, P <0 0 5; posterior ciliary artery blood flow spectrum peak backward, the peak dull deformation vascular hyperresponsiveness changes. At the same time, IONP prognosis and internal carotid artery blood flow abnormalities were analyzed. Conclusion CDI can provide hemodynamic basis for the diagnosis and treatment of clinical IONP. For the clinical study of retina and optic neuropathy physiological blood microcirculation provide effective information.
