目的　介绍颅颈交界处肿瘤切除的经验 ,探讨远外侧入路的选择及显微手术技巧。　方法　对 2 3例患者采用远外侧入路 ,在显微镜直视下切除肿瘤。　结果　肿瘤全部切除 17例 ,近全切除 4例 ,大部切除 2例 ,全切除率74% ,无死亡。　结论　远外侧入路可以非常直接地显露下斜坡 ,易于控制椎动脉 ,对脑神经及颅颈交界处提供良好暴露 ,最小程度牵拉神经血管结构 ,使手术更加安全和成功 Objective To introduce the experience of tumor resection at the craniocervical junction and explore the choice of distal approach and microsurgical skills. Methods Twenty-three patients were treated by far-lateral approach and the tumors were excised under microscope. Results All the tumors were resected in 17 cases, 4 cases underwent total resection, 2 cases were mostly resected, and the total resection rate was 74% without death. Conclusions The far lateral approach can reveal the downhill very directly, make it easy to control the vertebral artery, provide good exposure to the cranial nerves and craniocervical junction, and minimally pull the neurovascular structures to make the operation safer and more successful
