目的探讨CT图像对颌骨囊性病变的诊断价值。方法选取经病理证实的颌骨囊性病变患者78例,包括面裂囊肿5例,含牙囊肿15例,根尖囊肿15例,牙源性角化囊性瘤27例,成釉细胞瘤16例。患者均行CT平扫检查,回顾性分析CT图像,观察病变部位、形态、分房情况、含牙个数、病变区牙根改变、周围骨质及临近结构侵犯情况。结果 78例中,囊肿发生在上颌骨30例,下颌骨48例。5例面裂囊肿表现为均匀低密度单发囊肿,牙根有推移但无骨质破坏。15例根尖囊肿表现为均匀的单发囊肿,无牙根及骨质破坏,但密度不均3例。15例含牙囊肿表现为单囊,均含有1颗及以上的未萌牙,1例出现骨质破坏。27例牙源性角化囊性瘤单囊型18例,多囊型9例,囊腔密度多均匀。16例成釉细胞瘤中多囊型10例,囊实性改变14例,骨皮质缺损15例,牙根骨质破坏11例。结论 CT图像对颌骨常见囊性病变的诊断及鉴别诊断具有重要意义,能够为临床治疗提供依据。
Objective To investigate the diagnostic value of CT in cystic lesions of the jaw. Methods Totally 78 patients with pathologically confirmed cystic lesions of the jaw were included, including 5 cases of facial dehiscence, 15 cases of dental cysts, 15 cases of apical cysts, 27 cases of odontogenic keratocysts, ameloblastoma 16 example. All patients underwent CT plain scan. The CT images were retrospectively analyzed. The lesion location, shape, delivery situation, the number of teeth included, the change of the root area of the diseased area, the surrounding bone and adjacent structures were observed. Results In 78 cases, the cyst occurred in 30 cases of maxillary and 48 cases of mandible. Five cases of dehiscence showed uniform low density single cyst, there is a gradual root but no bone destruction. 15 cases of apical cysts showed uniform single cyst, no root and bone destruction, but the density of 3 cases. Fifteen cases of dental cysts showed single cyst, both of which contained one or more non-adorable teeth, and one case had bone destruction. 27 cases of odontogenic keratocystic neoplasia in 18 cases of single capsule, multi-capsule type in 9 cases, more uniform cyst density. 16 cases of ameloblastoma multiplastic 10 cases, cystic change in 14 cases, 15 cases of cortical defects, root destruction of 11 cases of bone. Conclusion CT images are important for the diagnosis and differential diagnosis of common cystic lesions in the jaw, which can provide the basis for clinical treatment.
